An Affidavit of Birth Certificate can be made by a person who has detailed personal knowledge of the facts of the birth certificate of the person whose birth needs to be proved. It is preferred that the Affidavit of Birth Certificate be made by an older blood relative, although it may be made by the attending physician or any other person who has intimate personal knowledge of the birth.

A notarized Affidavit of Birth Certificate with the correct information should be made by a close relative with evidence to the birth in question. If the parents are alive, they should make the Affidavit of Birth Certificate, but if this is not possible, a close relative who is older than the Applicant may provide the affidavit.

Remember that all statements in an Affidavit of Birth Certificate are sworn under oath in front of a notary public. Any falsification or misleading statements will result in heavy fines and punishment of perjury.
