The applicant must sign the Affidavit of Support I-134 form in his or her full name. Signing an Affidavit of Support I-134 form is upheld by penalty of perjury under United States law, so all applicants are thoroughly held accountable. For this reason, it is not necessary to sign an Affidavit of Support I-134 form in the presence of a notary or to have your signature notarized after you sign the Affidavit of Support I-134.

The National Visa Center reviews all Affidavit of Support I-134 forms for completeness and correctness. When a corrected Affidavit of Support I-134 form is accepted by the National Visa Center, it will be sent with the immigrant’s visa petition to the embassy or consulate where the applicant will apply for a visa.
